To begin the enrollment process, please complete the Inquiry Form below. This form is for prospective students entering Preschool through 2nd grade.  

Please note: We are nearly at capacity in grades 3–8 and are not currently accepting new enrollment inquiries for those grade levels. After you submit the form, our team will contact you with next steps.

Important Information

Submitting the Inquiry Form does not officially enroll your student at Holy Name School. This form is Step 1: an inquiry. Official acceptance and registration will take place later and depends on class size availability and review of academic and behavioral records.

Age Requirements (by July 31)

• Preschool: Child must be 3 years old by July 31
• Pre-Kindergarten (Pre-K): Child must be 4 years old by July 31
  • Students entering Pre-K must be fully potty trained
• Kindergarten: Child must be 5 years old by July 31
